Voluntary Vision Plan

The Personal Benefits Plan provides a vision plan based on group rate premiums. Vision Service Plan (VSP) is the largest vision care provider in the United States.

Vision coverage is offered as an optional benefit therefore, if you choose to purchase it, you will be responsible for the premiums for you and your dependents.

Vision care is covered according to an In-Network or Non-Network rate structure. The vision plan covers charges by physicians outside the VSP network, but at a lower rate. Also, there is a provision in the policy that states that all non-network claims must be made within six months of services rendered.

A brochure containing a list of all the participating in-network physicians is available in the Human Resources office at each Baptist Health facility. If your current physician is not in-network, you may contact him/her to see if he/she is interested in becoming a member and VSP will contact your physician to add him/her to the list.

There is no pre-existing condition provision for vision coverage. You may change your vision benefit selection each year. You may choose either glasses or contacts each year.
