Voluntary Cancer Plan

Another optional benefit available to employees is Cancer/Dread Disease Insurance. This coverage, offered by USAble Life, will pay for the actual charges for treatment in the hospital, a clinic or your physician’s office for skin cancer, internal cancer, leukemia, Hodgkins Disease or any other type of cancer (subject to certain conditions).

All cancer benefits are paid directly to you, in addition to any other medical insurance you may have. This coverage is not automatic. You must complete a questionnaire to be approved for coverage. This coverage may not be available if you, your spouse or dependents have had internal cancer, or certain other types of cancer.

Radiation Therapy/Chemotherapy/Immunotherapy’s actual charges are covered up to $5,000 per year.

The Cancer/Dread Disease Insurance option also pays benefits for hospital stays relating to dread diseases. The benefit pays actual charges up to $250 per day for hospital confinement due to other specified diseases. A complete list of these diseases is available in Human Resources or through your department benefits “expert”.
