Group Life and AD&D Plan

Group Term Life Insurance is an insurance policy offered to you, and paid for, by Baptist Health as long as you are an employee. The benefit is one-times your base salary. In other words, if your annual, base salary is $30,000, Baptist Health will provide you with $30,000 of group term life insurance as long as you remain an employee.

This benefit is provided to all full-time employees (authorized to work 70 or more hours a pay period). If you are a part-time employee (authorized to work 40-69 hours per pay period), you may purchase any level of Term Life and core AD&D coverage at your own expense.

Accidental Death & Dismemberment (AD&D) is a benefit that pays in the event of a dismemberment or accidental death. Dismemberment refers to the loss of a limb, sight, speech, hearing, etc. Dismemberment benefits are paid to you at 100%, 50%, or 25% of the insurance value – based upon your loss. See your Benefit Guide or actual insurance policy for a complete payment schedule.

Additional Coverage Features:

  • Your life insurance plan pays double the benefit amount in the case of accidental death and a percentage of the selected coverage for dismemberment. Accidental death means – “an unexpected, unplanned event”, not as a result of failing health.
  • The waiting period for benefit eligibility for Term Life/AD&D is 30 days.
  • Additional AD&D coverage is available to you, at group rates, through USAble Life. This coverage is optional. Therefore, you are responsible to pay the premium for this additional coverage, which is funded with pre-tax dollars.
  • You may choose any beneficiary (the person designated to receive the benefits in case of your death) you desire, and may change the beneficiary any time during the year by contacting the Human Resources office at any Baptist Health facility.
  • Retirement: If you retire before age 65, your life insurance terminates when you retire. If you retire at age 65 or after, life insurance benefits reduce to 50% of your salary at retirement, and to $1,000 at age 70. If you continue to work full-time to age 65 and beyond, benefits continue with no reduction. AD&D benefits terminate at retirement.
  • This policy is NOT PORTABLE. It will NOT go with you when you terminate from Baptist Health. You may want to consider additional, private insurance.
