Group Dental Plan

* You select medical and dental coverage separately. You can elect dependent coverage in either plan or both plans.

* There is a 6 MONTH service waiting period for all orthodontia benefits from your effective date of coverage.

* Preventative care coverage covers the following:
- routine examinations - limited to twice annually
- diagnostic x-rays
- full mouth and panoramic x-rays - once in 3 years
- cleaning, polishing - twice per year

* Basic care includes dental procedures such as:
- fillings, extractions, anesthesia, specific oral surgery, etc.

* Major care coverage includes dental procedures such as:
- crowns, dentures, and partial dentures

* Orthodontia Care is treatment for correction of malposed teeth of eligible dependent children under age 19. Orthodontia treatment is the establishment of proper occlusion through the movement of teeth or their maintenance in position. Employees and their spouses are NOT ELIGIBLE for orthodontia coverage.
